Documents Needed For The Application Of Your Loan
Print out the list below to help keep track of what you need.

  • W-2 Forms for the previous two years
  • Pay Stub - for most recent one month’s earnings
  • Most recent statements for bank accounts, mutual funds, and retirement accounts -all pages
  • Personal tax returns for the previous two years -all pages
  • Corporate or Partnership tax returns for previous two years -if you are self employed
  • Driver's license and social security card
if applicable
  • Complete copy of bankruptcy papers including discharge list.
  • Divorce decree and/or separation agreement
  • Veteran - Certificate of Eligibility and DD214

The Department maintains a recovery fund to make payments of certain actual out of pocket damages sustained by borrowers caused by acts of licensed residential mortgage loan originators. A written application for reimbursement from the recovery fund must be filed with and investigated by the department prior to the payment of a claim.
